Application Instructions
The Application. A
completed
application consists of an application form, an unofficial transcript, and two letters of recommendation, all of which
need to be submitted electronically.
- Student
Application Form. Please fill out this form completely; only fields identified as optional
may be left blank. It will ask you for personal information; contact information; academic information including the
dates of your 2010 spring break, the last day of classes at your home institution, and a list of all the college
math courses you've taken with your grade and textbook used; recommender information including
the names, titles, emails, and phone numbers of your two recommenders;
and finally your statement of interest. Your statement of interest is important to us; please write and edit your statement elsewhere
and cut-and-paste it into this form. In your statement, briefly and candidly tell us about your interest in mathematics or the mathematical
sciences, and indicate why you would like to participate in our program. Please limit your statement to 500 words.
- Transcript. Upload a transcript showing courses and grades
from the
institution you are currently attending, including grades from fall
term
2009. Unofficial copies of transcripts are encouraged. You can, for example, download an unofficial transcript from your college or
university into a .pdf format, and upload it here. Your application needs to be submitted before you can upload your transcript.
- Faculty
Recommendations. Two letters of recommendation, preferably from
college or university mathematics instructors, are required. The recommenders will need to know which email address you are using
on your application so that their recommendation gets linked to your file. Your application needs to be submitted before your recommenders
can write their letters.
